Please arrive 10-15 mins prior to your appointment time. Parking is limited to metered street parking or the downtown parkade 2 blocks east of our clinic.

For your first visit, we will ask you to fill out a comprehensive medical questionnaire that will include specific questions on your medical conditions, current medications you are taking and any previous surgeries you have had. The questionnaire will either be emailed to you via Ocean MD, or can be completed on the day of your appointment.

 

Appointment Cancellations - We require 24 hours notice prior to your appointment to cancel a clinic visit without penalty. Please call the clinic (250-545-6443) and either speak to a staff member or leave a message to cancel. A no-show charge will apply if you do not show up for your appointment.
